These terms and conditions ("Agreement") govern your engagement and use of the home care services provided by Lassad Care Services. By engaging our services, you agree to be bound by these terms and conditions.

1. Service Provision: Lassad Care Services agrees to provide home care services to the best of its abilities, taking into account the specific needs and preferences of the client. The services provided may include personal care, companionship, assistance with daily activities, and other agreed-upon services as outlined in the care plan.

2. Client Responsibilities: As the client or authorized representative, you agree to provide accurate and complete information regarding your needs, preferences, and medical history. You also agree to cooperate with Lassad Care Services and its caregivers to facilitate the provision of home care services.

3. Care Plan: Lassad Care Services will develop a care plan tailored to the client's specific requirements. The care plan outlines the services to be provided, the schedule of care, and any special instructions. The care plan may be subject to reasonable modifications as needed, with the agreement of both parties.

4. Caregiver Assignment: Lassad Care Services will assign caregivers based on availability and the client's needs and preferences. While we strive to ensure consistency in caregiver assignments, we reserve the right to make changes when necessary, such as due to caregiver availability or emergencies.

5. Fees and Payment: You agree to pay the fees for the home care services as outlined in the agreement or as otherwise communicated to you. Fees may vary depending on the type and duration of services provided. Payment terms and methods will be agreed upon and communicated prior to the commencement of services.

6. Confidentiality: Lassad Care Services acknowledges the confidential nature of the client's personal information and will maintain strict confidentiality in accordance with applicable privacy laws and regulations. Personal information will only be disclosed as necessary for the provision of home care services or as required by law.

7. Liability and Insurance: Lassad Care Services maintains appropriate liability insurance coverage for its caregivers. However, we recommend that clients maintain their own insurance coverage to protect against any potential liability related to their property or personal circumstances.

8. Termination: Either party may terminate this agreement upon written notice, subject to any applicable minimum notice periods or conditions specified in the agreement. Lassad Care Services reserves the right to terminate the agreement in cases of non-payment, non-compliance with these terms and conditions, or any behavior that endangers the safety or well-being of its caregivers.

9. Limitation of Liability: Lassad Care Services will exercise reasonable care and skill in providing home care services. However, we shall not be liable for any direct, indirect, incidental, special, or consequential damages arising from the provision or use of our services, except where prohibited by law.

10. Governing Law: This Agreement shall be governed by and interpreted in accordance with the laws of the jurisdiction in which Lassad Care Services operates.

11. Entire Agreement: This Agreement constitutes the entire understanding between the parties and supersedes any prior agreements or understandings, whether written or oral, relating to the subject matter herein.
