How supported living plans are created.
We will learn and understand each person's history, their current situation and what their plans are. We work along side, family members/friends, care professionals to make sure we have the necessary information and in-depth information that we will have gathered. Once a supported living is in place, we do regular reviews on progress, encourage and maintaining the encouragement, set more achievable goals.
